Who is Eligible?

Community-Based and Individual Supports are designed for adults with intellectual and/or developmental disabilities who face challenges functioning at home or in the community. These programs are funded through Medicaid Insurance, and individuals must be eligible for the Division of Developmental Disabilities (DODD).

To assess your eligibility for Community-Based Services (CBS) or Individual Supports (IS), please reach out to your current support coordinator. Freedom Family Home Healthcare is here to collaborate with you and your family, and you can request our services to provide the support you need.

How to Obtain CBS or IS

If you believe these services are suitable for your loved one, please reach out to your support coordinator and express your interest in in-home supports, specifying that you would like Abilities to provide these services.

Once we receive a referral from your support coordinator, our staff will follow up with any necessary questions to ensure we can provide the best support for your loved one.

After acceptance for in-home supports, your support coordinator will include Community-Based Services (CBS) or Individual Supports (IS) in your Individualized Service Plan (ISP) and Service Delivery Record (SDR).
